Terry Goodkind made Denna and the Mord-Sith this way on PURPOSE, to show just how fucked up that D'Hara is under Darken Rahl. Either be tortured, or be tortured slightly less and have some pleasure thrown in the mix, if you are a good pet of course. Denna even goes as far as to say if he's a good pet, he can even be her mate and get to enjoy those privileges. She even tells Richard to think pleasant thoughts of her, obviously alluding to something sexual. This is obviously a conscious decision, and explains why Denna is so good at training people compared to others like Constance etc., who only uses pain and torture. She not only hurts him, and degrades him in public, but the also uses her position as a very attractive woman in tight leather to get in his head and humiliate him. She also knows that if her goal is to 'break' a man, using sexuality is an even more effective way to do it.
Denna dresses up in a skintight red leather outfit, because hey, she's hot, and she knows it. Torture, humiliation, degradation and rape are real things that are used by evil people in the real world. Rape is a very sensitive topic for many people, yet I don't see why this scene gets so much hate in particular compared to other book series or shows like Game of Thrones etc. The torture scene in the first Sword of Truth book is quite brutal.
